SQL Server 6.5 Violation of PRIMARY KEY

 SQL Server 6.5 Violation of PRIMARY KEY

Dear all,

We are working with SQL Server 6.5 and  when we try to
insert a record into a table called 'tblAbonneenummer', we
get the following errormessage:

 Microsoft OLE DB Provider for SQL Server error '80040e14'
Violation of PRIMARY KEY constraint 'PK___2__12': Attempt
to insert duplicate key in object 'tblAbonneenummer'.

This table contains two Columns. A colomn which holds
integers and one that holds Varchars. Te column that holds
integers has a primary key and is an indentity field
(identity seed = 1, identity increment = 1). The index has
a fillfactor of 90%.

Do you have any idea what could cause this error. SQL
Server should make a new unique id when inserting a
record, but this does not seem to happen. The error occurs
when trying to insert a record manualy and when using
ASP/ADO.
