Can anybody out there convert the information in a web form to a
preformatted MS word document and store all the the form info in a
database??

i.e. user fills in there name, address, age, etc. They receive an e-mail
with the word document attached, and all the info is stored in a database

This is a project that will begin soon. Please respond asap by e-mail
