 Sql Server 6 & Identity columns

My apologies if this has been asked before.

db grids and a table that uses an identity column
When a sql select returns no records to the grid (not
  because the table is empty, but the result set for this key is empty)
and a user inserts a record - Two appear in the grid!

One assumes that the first is the one without an identity and the second
is the one with the identity.  Requerying the server makes this "shadow"
go away and it will not happen again for this key.  All following inserts
are "singles" and not doubles.  I killed off the effect with icky code
but was wondering what the suggested approach would be?
